Registered Nurse (RN) – Home Health

Job Overview

We are seeking a compassionate and experienced Registered Nurse (RN) to join our Home Health team. The RN will be responsible for assessing patients, implementing individualized care plans, providing case management, and coordinating care to promote health maintenance, disease prevention, and improved patient outcomes within the home setting.

Schedule

  • Monday – Friday, 8:00 AM – 5:00 PM
  • On-call rotation: 1–2 weekdays per week and approximately one weekend per month
  • 40 hours are not guaranteed

Territory Covered

  • Clinton County
  • Cumberland County
  • Monroe County

Compensation & Benefits

  • Mileage reimbursement: $0.43 per mile

Required Qualifications

  • Active RN Compact State License
  • Home Health experience required
  • Experience with HCHB (Homecare Homebase)
  • OASIS assessment experience
  • Strong case management and care coordination skills

Required Clinical Skills

  • IV therapy
  • Wound care
  • PICC line management
  • Wound VAC management
  • Tracheostomy (Trach) care
  • Lab draws

Productivity Expectations

  • 20–30 productivity points per week

Key Responsibilities

  • Assess, plan, implement, and evaluate nursing care for patients in the home setting.
  • Develop and manage individualized care plans to support patient health and independence.
  • Provide case management and coordinate care with physicians, therapists, and other healthcare professionals.
  • Monitor patient conditions and communicate changes to physicians promptly.
  • Complete OASIS assessments and maintain accurate documentation in HCHB.
  • Administer and monitor medications, treatments, and nursing interventions.
  • Educate patients, families, and caregivers on disease management, medications, and safety practices.
  • Coordinate health-related appointments and ensure timely follow-up on recommendations and findings.
  • Maintain communication with hospitals, discharge planners, and other providers during care transitions.
  • Supervise and provide guidance to LPN/LVNs and other support staff as needed.
  • Ensure compliance with infection prevention, medication management, and regulatory requirements.
  • Complete all required daily, monthly, quarterly, and annual nursing documentation.
  • Participate in staff training, health and safety education, and ongoing quality improvement initiatives.
